Unfortunately, over the coming weeks, our pharmacist continues to struggle and makes quite a few errors with the discharges. The GPs at the practice are disappointed because they feel the task of medication reconciliation should be very straight forward for a pharmacist.
After 3 significant near misses, the GP partners decide to have a discussion with the pharmacist. Two more weeks elapse and the pharmacist makes 3 more simple errors in the discharge process. At this point, the GP partners decide to report our pharmacist to the GPhC.
The pharmacist is understandably devastated and really upset at being reported to the regulator.
